New Patient History Packet TMD
If you are being seen for a follow-up appointment, please arrive approximately 10 mins early to get checked-in properly. In the first year of treatment, one should expect 4-5 visits to make sure the treatment is therapeutic and to allow the patient to express any needs or concerns. Patients will have labs drawn at every appointment. After the first year, most patients are seen every 6 months.
Payment | Cost
Patients should expect to have an insurance card on hand at every appointment. Patients with insurance should expect to pay a co-pay at the time of each visit. Cash and credit cards are accepted. For those who would like to have cash options and not use insurance plans, payment options are available as well.
